Either one or both of the programs together may have caused behavioral change or at least sensitized IV drug users to the need for such change (Moss and Chaisson, 1988). To make rational decisions about the kinds and amounts of resources to be directed toward drug-use problems, the government needs quantitative information on the size of those problems. As of November 14, 1988, 20,752 cases of AIDS had been diagnosed in individuals who reported IV drug use (CDC, 1988).

Investigating Risk-Associated Drug Behaviors

As always, thoroughly clean the injection site with alcohol, soap and water, or other disinfectant prior to injection. If there’s too much blood in your shot to tell if you’re properly registering, split the shot into two and dilute each half with water. If the plunger of your syringe is forced back by the pressure of the blood and it is bright red, frothy, and ‘gushing,’ you may have hit an artery. Perhaps the safest way to learn how to inject is to have someone who knows what they’re doing teach you.

However, the sterilization of injection equipment is not without problems, as some disinfectants may dissolve the silicone lubricant of the syringe plunger, thus making its operation quite stiff. This is therefore a variant of the common method of injection with a dropper with the hypodermic needle affixed, using a “collar” made of paper or other material to create a seal between the needle and dropper. Removing part of the plunger assembly by cutting off most of the shaft and thumb rest and affixing the bulb to the end of the barrel, thereby allowing the bulb to operate the plunger by suction, also does work in many cases. Specific treatment depends on the drug being used, but it typically involves counseling and sometimes involves use of other drugs (for example, methadone as a safer drug to replace heroin). Family support and support groups help people remain committed to stopping use of the drug.

Medical Complications of Injection Drug Use

Doppler ultrasound demonstrating diffusely increased flow indicative of hyperaemia (Fig. 1) helps differentiate cellulitis from non-infectious oedema, in addition to clinical history and examination 9. Computed tomography (CT) in cellulitis demonstrates increased attenuation and stranding of the subcutaneous fat, due to oedema, with overlying skin thickening. However, criminalization of drug use and stigma and discrimination against people who inject drugs contribute to ongoing epidemics of HIV, viral hepatitis and TB. This is because people who inject drugs fail to access harm reduction and other health services.

These “invisible” drug users are apt to include individuals in the mainstream of society (i.e., from upper and middle socioeconomic classes) who are more likely to seek treatment in private health care facilities. Still, significant research has been conducted and data of variable quality have been collected, but topics remain that are either underexplored or that have not been investigated at all. Most studies on IV drug use have investigated the injection of heroin; much less is known about the injection of cocaine or other drugs. The methodological problems are as diverse as the population to which drug researchers wish to apply methodologies. Drug users do not necessarily cooperate as research subjects by restricting their behavior to forms that can be studied using simple questionnaires. For example, single-substance drug use lends itself relatively easily to research design; the polydrug use that a significant portion of IV drug users actually report is much more difficult to measure (B. D. Johnson et al., 1985).

Geographic variation in the number of AIDS cases indicates that there are still important opportunities to prevent the further spread of the disease. The following paragraphs discuss what is known about HIV infection and describe the patterns that have been seen to date. Dual-systems estimates, also known as capture-recapture or tag-recapture estimates, are perhaps the most widely used method for making direct estimates. The capture-recapture method assumes that only a small fraction of the addict population would ever be reported to authorities. Two independent lists of IV drug users are generated—for example, a list of persons in heroin treatment programs and a list of persons treated in emergency rooms for adverse reactions to heroin use.

However, several viral infectious diseases are still transmitted between IV drug users, including hepatitis C (HCV), hepatitis B (HBV), and HIV/AIDS. Aftercare is a crucial component that involves ongoing support and resources to help individuals maintain their recovery in the long term. Support groups, individual counseling, and community-based programs contribute to the continuity of Drug rehabilitation care and the prevention of relapse.

Understanding IV Drug Use

When injecting a drug intravenously, the individual introduces a bolus of this substance into the vein, producing a rapid and powerful drug high. The onset of drug effects is about 15–30 seconds for the intravenous route and 3–5 minutes for the intramuscular or subcutaneous route. Drug effects from inhaling (ie, smoking) a substance can begin in 7–10 seconds and drug effects from intranasal use (ie, transmucosal absorption) can begin in 3–5 minutes. iv drug use To help prevent infection, the administration process must be done carefully using sterile (germ-free) equipment. Key populations at higher risk of HIV include people who sell sex, men who have sex with men, transgender people and people who inject drugs.
